Neurocognitive Recovery with Amantadine in Intensive Care Unit Patients with Head Trauma: Case Series

چکیده
Kafa travması nedeniyle oluşan travmatik beyin hasarı (TBH) yüksek morbidite ve mortalite ile seyreden ciddi bir sağlık sorunudur. TBH'de tedavi arayışları sürecinde nöroproteksiyon sağlamak amacıyla birçok farmakolojik ajan uygulanmıştır. Amantadin, N-metil D-aspartat reseptörlerine antagonist etkisi yanında dopaminerjik reseptörlere de direkt agonistik etkisiyle sağlamaktadır. Yoğun bakım ünitesinde, Glasgow Koma Skoru (GKS) 10'un altında olan 16- 72 yaş arasındaki 5 kafa travmalı hastaya standart tedaviye ek olarak amantadin protokolü (ilk hafta 200 mg/gün intravenöz infüzyon şeklinde, 2. enteral yoldan, 3 4. haftalarda ise 400 yoldan) uygulandı. Amantadin tedavisi GKS Güncellenmiş Komadan Çıkma Skalası skorlarında artış, Sakatlık Derecelendirme Ölçeği skorunda azalma saptandı. Sonuç olarak, TBH gelişmiş hastalarda amantadinin klinik nörofonksiyonel iyileşmeye katkı sağladığını düşünmekteyiz.
